Promoting and Coordinating Healthy Lifestyles

The Care Connections Program* was developed to help promote and coordinate patients’ needs within the Patient Centered Medical Home (PCMH). Care Coordinators educate patients and their families about healthy lifestyles to meet their individual health goals, assist with access to community services, and to promote well-being.

The Care Connections Program connects patients and their families to appropriate community resources. Our mission is to optimize your health and well-being by understanding what is important to you. We utilize a team approach that includes your primary care provider, care coordinators, and community partners to help you accomplish your goals. We measure the success of our program based on patient satisfaction, personal wellness, and nationally recognized benchmarks of quality.
Our Care Coordination Team (RN/MA) works with you in:

  • Setting healthy goals for optimal weight, blood sugar, and blood pressure
  • Disease management and education
  • Communication across the PCMH including primary care providers, Community Memorial Hospital, and community resources
