Dr. Paul Cisarik received his medical degree from Saint George’s University School of Medicine, completed his residency at MetroHealth Medical Center, and his master’s degree in nutrition from Case Western Reserve University. He is a diplomate of the American Board of Internal Medicine and the American Board of Obesity Medicine. Dr. Cisarik specializes in internal medicine with a focus on nutritional therapy. This includes medically supervised weight loss, management of medical and nutritional issues before bariatric surgery, as well as long-term care after bariatric surgery. He also provides nutritional support for disorders of malnutrition, including treatment of nutritional deficiencies and dietary counseling in relation to different gastrointestinal disorders. Dr. Cisarik speaks English, Spanish, Slovak and conversational Serbo-Croatian.
Dr. Clayton received her medical degree from Jefferson Medical College in Philadelphia, and completed her internship and residency in internal medicine at Northwestern Memorial Hospital/Jesse Brown Veteran’s Affairs Hospital in Chicago.
Ms. Kristine Supple, PA-C, MPH is the director of population health, ambulatory medicine at Community Memorial Health System. She has expertise in value-based program development, clinical analytics, driving population health initiatives and direct patient care. She developed and provides leadership for the Care Connections Department, where her clinical team provides care management, disease management and chronic care management services. Ms. Supple has a physician assistant degree from George Washington University School of Medicine and an MPH degree in health care management from the Yale School of Medicine and has been caring for internal medicine patients for over 20 years.
Dr. Shaun Patel specializes in cardiology. He received his medical degree from St. Louis University School of Medicine and completed an internal medicine residency, cardiovascular medicine fellowship and interventional cardiology fellowship at Banner Good Samaritan Medical Center in Arizona. Dr. Patel is a member of the American Medical Association, American College of Physicians and American College of Cardiology.
Dr. Michelle Levin received her medical degree from New York Medical College. She completed residency training in internal medicine at OV-UCLA Medical Center in Sylmar and an endocrinology fellowship at UCSD. Dr. Levin is certified by the American Board of Internal Medicine.
Dr. Laya Nasrollah specializes in gastroenterology. She received her medical degree from University of Texas Medical Branch in Galveston. She completed an internal medicine residency at University of Arizona, and a gastroenterology fellowship at Banner Good Samaritan Medical Center in Arizona.

She received her medical degree from St. George's University School of Medicine in Grenada, West Indies, completed a residency in categorical internal medicine at East Carolina University, and a rheumatology fellowship at Cedars Sinai Medical Center. Dr. Shepard is board certified and is a member of the American College of Rheumatology.
